A: INTRODUCTION:

1. Sanjay Gandhi Memorial Hospital was commissioned in April 1986 as one of the seven 100 bedded hospitals planned by the Govt. of NCT of Delhi during the 6^th five year plan under Special component plan for Schedule Cast/Schedule Tribes. Later it was augmented to 300 beds.in 2010

2. The hospital now caters to the health needs of a population of 15-20 lakh residing in the JJ clusters & resettlement colonies of Mongolpuri, sultanpuri, Nangloi, Mundka & Budh Vihar etc.

3. The Hospital is providing free health are facilities of OPD, casualties,Indoor & Emergency Services of all common specialities along with free drug distribution and investigations for patients. At present it is managing a patient load of about 2500 OPD patients per day and bed Occupancy 125%.

SGMH is located at S- Block Mangolpuri, and is 3 Km. from Peera Garhi chowk. It is easily accessible by bus route numbers 568, 569, 808, 939, 940, 943, 944, 953, 959 and 982 by local means of transport.

Q.1) Who can avail facilities of the Hospital?

Ans: Anyone in need( Indian or Non-Indian) can avail the facilities in the hospital

Q.2) What facilities are available?

Ans: At present OPD (Medicine, Surgery,Obstetrics and Gynecology,ANC,Family,Welfare,Immunization,Pediatrics,Orthopedics,Eye,ENT,Skin & V.D,Dental Dot Center), Investigations,routine operations,Facility for Delivery and emergency opertation for surgery,orthopedics,Obs & Gynae,Major and minor surgical operations in surgery,Orthopedics,Obs.& Gynae,Eye and ENT are performed.

CT scan for patients admitted in casualty and emergency(outsourced).

Q.3) How can one avail hospital facilities?

Ans: A patient has to come to hospital, as home visit or home ambulance services are not available at present. For availing O.P.D. consultation he/she has to get himself /herself registered and get his/her O.P.D. card made .After registration, he goes to concerned doctor and gets free consultation,investigations if required and medicines from hospital pharmacy.In emergency he/she has to come to Casualty Department where he/she gets medical treatment for immediate relief and advised admission/ operation or other treatment if needed .All services are provided free of cost .For ultrasound investigation a nominal fee of Rs 50/- is charged.

Q.4) When can one avail facilities?

Ans: Routine health care services (Consultation, Investigation and Medicines) are available on all working days (that is all days except Sundays and Gazetted holidays), after getting O.P.D. Registration whose timings are 8.30 A.M. to 12.30 P.M. (Weekdays) and 8.30A.M. to 11.30 A.M. (Saturday). However in case of any emergency, medical facilities are available in Casualty Department round the clock.

Q.5) How much waiting time it takes for person to avail facility in hospital?

Ans: On normal day it takes about 10-20 minutes for registration, 10-20 minutes for consultation and 20-30 minutes in getting medicines and more time is required if investigations are also done on the same day.

Q.6) Who can avail Investigative facilities of the hospital and when?

Ans; Investigative Facilities are provided only on advice of treating doctor. Routine registrations (Laboratory and X-Ray Ultra Sound Services etc.) is done from 9A.M. to 11 A.M. (Report are usually availabe on the next working Day), whereas urgent investigations are done round the clock only for Admitted and Casualty patients. Ultrasound study is done with prior appointment.CT scan is available only for patients admitted in casualty and emergency.

Q.7) How can a patient get operated?

Ans: If a patient comes in casualty and if immediate operation is essential,then doctor operates him/her in emergency operation theatre, which is open 24 hours. If patient comes in OPD,he is checked for fitness for surgery by getting pre-anesthetic check up after routine investigation and given appointment for operation depending on the deparments workload and waiting time.

Q.8) What are Blood Bank facilities?

Ans: Blood bank issues blood only to those needy patients who are admitted and treated in this hospital . Blood bank is open for Voluntary donation from 9 A.M. to 4 P.M. and a Donor card is issued by Blood Bank against which donor can get blood for him/her or close relative for one year. He/she can also get results of HIV, HBSAg, HCV, VDRL or Malarial infection of his blood if he wants.

I: Facilities not available:

Nursing Home, Burns ward, Doppler, Echocardiography, T.M.T., Holter, Dialysis, Super speciality consultation,C.T Scan (Except for patients admitted in casualty and emergency).

J: Citizen Charter

1. Facilities:

	All facilities in this hospital are free of Cost except Ultrasound which is charged at Rs.50/= per case.(Ultrasound is free for pregnant women and indoor patients).
	O.P.D. registration has separate queues for male, female, senior citizen and Freedom fighter, persons with disabilities patient and Delhi govt.Health Scheme beneficiaries.
	O.P.D. Registration Timings: Weekdays 8:30 A.M to 12:30 P.M.,
	Saturday 8:30 A.M. to 11:30A.M
	Investigation Registration Timing: Weekdays 9 A.M. to 11 A.M.
	Saturdays 8:30 A.M. to 10:30A.M
	All the medicines are given from Hospital Pharmacy free of cost and are usually given for three days but for illness requiring prolonged treatment,medicines are given for 15 or 30 days.
	Indoor patient is provided all facilities free of cost with free food
	Free Ambulance is available for referring needy patients to specialized hospitals

2. Other facilities available are:

3. Responsibility of the patients and their attendants:

	Observe all instructions as they are for your quick recovery.
	Do not smoke as whole hospital campus is“No Smoking Zone” Do not spit.
	Keep hospital and its surroundings clean and throw garbage in designated dustbins placed at various convenient places.
	Maintain silence in hospital.
	Don’t behave in a manner, which causes inconvenience to other patients.
	Don't pay for any services to anyone as all services are free of cost except ulta sound investigation.
